About Pyruvate Kinase Deficiency post Splenectomy

This blood film shows all the features of pyruvate kinase deficiency, a hereditary enzymopathy, and those of a patient post splenectomy. 

There are two main red cell enzymopathies which can be diagnosed on a blood film - PK deficiency and G6PD deficiency. These two look markedly different, and should never be listed on a differential together. To say this implies you know little about the morphology of one, or both or these conditions. 

PK deficiency causes numerous echinocytes, or burr cells. In PK deficiency, a lack of the PK enzyme blocks glycolysis, causing a severe shortage of cellular ATP. This energy failure stops the sodium-potassium pumps from working, disrupting ion and water balance and altering the cell membrane to form spikes.

Splenic macrophages recognises these cells as being abnormal, and try to remove them from the peripheral circulation. This causes anaemia, driving reticulocytosis. This process is therefore a form of congenital haemolysis. In individuals who suffer from sever anaemia, splenectomy used to be a treatment as removing the splenic macrophages means the echinocytes return to a normal life span in the peripheral blood. In practice, this doesn't always work - these cells aren't particularly healthy with no intracellular ATP. Nowadays, we tend to use other treatments rather than splenectomy.

Post splenectomy, you see all the changes associated with lack of splenic macrophage function. In the peripheral blood there are Howell-Jolly bodies, thrombocytosis, pappenheimer bodies, spherocytes and reactive lymphocytes.

Despite the alarming looking film, this patient is actually quite well. They need to take prophylactiv vaccination and antibiotics because of the hyposplenism (although some centres are challenging the role of antibiotics - so don't quote me on that!).

Thrombocytosis

Thrombocytosis is a common finding post splenectomy. The splenic macrophages have a key role in regulating the number of both white cell and platelets.

Echinocyte or Burr cell

This is an echinocyte- numerous small spiky projections from the cell surface. They are caused as a direct result of intracellular ATP depletion and ionic/lipid imbalance. They are different to acanthocytes, which often have fewer but more prominent spikes.

Howell Jolly Body

Howell Jolly bodies are nuclear DNA remnants which would normally be clipped off by splenic macrophages. In a patient post splenectomy, or with functional hyposplenism, this doesn't happen.

As before, this is a DNA remnant. HJBs should be easy to find in a patient with anatomical or functional hyposplenism but will be rare in anybody with a functional spleen.

Pappenheimer Bodies

PAppenheimer bodies are abnormal iron granules seen inside red cells. They occur in many conditions, but are often more prominent in those with hyposplenism, where the splenic macrophages do not remove them.

Spherocyte

Spherocytes are again common in hyposplenism.

Howell Jolly Body

A particularly large HJB in this cell. HJBs are DNA remnants.

Reactive lymphocyte

This lymphocyte has a little too much cytoplasm and the chromatin in the nucleus a little too open to call it a normal, small mature lympocyte. It is reactive - a catch all for lymphocytes which are not normal but not malignant. These are common post splenectomy. Activated or reactive lymphocytes would normally be removed in the spleen.

Cluster of Echinocytes

This is a small cluster of echinocytes ot burr cells. Note that not every cell here is a burr cell - when the patient is having a crisis, there may be more echinocytes than normal, but the blood film can be pretty unexciting for a well patient.
